Procedural inadequacies

 

Consideration of procedural inadequacies covers situations where the defined procedures were clearly wrong or inadequate.

A classic example would be the Three Mile Island (TMI) event where the operators were instructed by procedures to do something that made the event worse.

Another example would be where maintenance procedures are found to be inadequate or important components are missing from a surveillance programme.
